Lab Software Installation Request Form

Use this form to request software installations in computing labs, electronic classrooms, presentation classrooms and conference rooms.

Software Installation Request Guidelines

• Windows 11 Compatibility
  All software must be certified for use with Windows 11.

• Proof of Licensing
  A valid software license is required. Please attach a copy of the software licensing agreement to this request.

  • If the software or vendor permits educational use, include a screenshot from the vendor’s website explicitly stating this.

• Submission Timeline
  Submit this request at least 4 weeks prior to:

  • The start of the semester when the software will be used, or

  • The first planned use of the software—whichever comes first.

  • This timeline is essential due to limited access to classrooms and lab computers during peak academic periods.

  • Failure to comply with these guidelines may result in the software not being available for use

• Software Media Requirement
  The software installation media (e.g., download link, installation files, activation keys) must be provided with this form

• Responsibility and Support

  • Faculty members requesting the software are responsible for supporting its use in their courses.

  • Information Technology will handle the installation and verify that the software functions as expected.

• Form Submission
  Complete all required fields in this form and click the Submit button at the bottom when finished.

Note on Plug-Ins and Course Websites

Faculty and Staff are responsible for ensuring that all plug-ins and web-based tools used in their course function properly. We strongly recommend testing all course-related websites on a lab computer prior to use. If you need assistance during testing, please reach out to lab staff for support.
