Service Level Agreement (SLA) For Professors
Make Up Exams
The purpose of the Office of Testing Services, Service Level Agreement (SLA), is to clarify the process of proctoring makeup exams to all parties involved. A make-up exam is any missed exam by an enrolled student in a UHD course of any modality. If a student qualifies for a make-up exam based on the course syllabus and college missed exam policy, the course instructor may proceed with completing the SLA form. The SLA form provides testing information for students and testing staff. Makeup exams are only available on Tues, Wed and Thurs. Testing Services will not provide make-up services to an entire class.
The following information is imperative and necessary for the Office of Testing Services to proceed with testing the student(s) on behalf of the course instructor:
- The SLA form must be completed by the course instructor
- The SLA form must be completed and the exam material must be submitted to testingservices@uhd.edu or dropped off in person in Suite OMB S530 a minimum of two business days in advance before the Office of Testing Services can reach out to the test taker for scheduling the makeup exam
- A new SLA (completed by the instructor) will be required for every request
- There is a two-student cap per request
- A new SLA (completed by the instructor) will be required when deadlines have expired
- Scheduling the exam date will only occur once all necessary information and items
to administer the exam are provided by the instructor to Testing Services (i.e.: exam,
password, charts, formulas, etc.).
- The instructor is responsible for notifying the student of approval for the makeup at the Office of Testing Services
- Students are responsible for monitoring their email from Testing Services, testingservices@uhd.edu for scheduling and information in regard to their makeup exam. All emails with scheduling options have an expiration deadline
- Testing Services will review all documentation and communicate with the student(s) to schedule their makeup.
- Instructors are required to provide Testing Services with a direct personal contact number (for office use only) for test-related issues that only the instructor can resolve
- University policy on duration is no more than 2.5 hours per exam
- Changes to the SLA form will not be accepted after submission
- A new SLA (completed by the instructor) will be required for every update/correction
- Please carefully review the SLA form prior to submission
- The SLA form will be reviewed the following UHD business day (excluding weekends, holidays and final exam week)
Office Campus Testing for Make-ups:
Students outside of the Houston metro area who are unable to come to UHD for testing purposes, if approved by their instructor and college, may utilize ProctorU or find a suitable proctoring (offsite) location near them.
- ProctorU is a third-party remote monitoring company that is available for a fee (fees vary based on duration and time requested).
- Students choosing ProctorU test monitoring services must pay and arrange their exam with ProctorU according to their instructor deadline.
- An offsite location requires a proctor agreement form and will need to be filled out by the student and the proctor. Testing Services will evaluate the request for In addition, Testing Services has a list of pre-approved testing centers available upon request.
