Reporting Options

The reporting options below provide University of Houston - Downtown community members an opportunity to share important information directly with appropriate offices. This is an internal reporting tool intended to address concerns in a safe, timely, and supportive manner, with the goal of connecting individuals to the resources they need.

  • Care Form - This form for reporting student issues related to mental distress, emergency financial need, the death of a loved one, hospitalizations, incarceration, health challenges, etc.
  • Student Complaint Form - This form to report issues related to any grievances experienced, processes, or procedures by university departments or personnel.​
  • Incident Report Form - This form for reporting student issues related to any violations of campus policy such as threats, alcohol, drugs, disruptive activity, etc.
