Employees, students, and anyone who does business with the University of Houston-Downtown can report fraud, waste, abuse, non-compliance, or misconduct (such as sexual abuse) by visiting the Fraud & Non-Compliance Hotline or call toll-free 1-833-841-4512. If the matter involves sexual misconduct, your report will be referred to the Title IX Coordinator.
File a Report via the Online Fraud & Non-Compliance Hotline
The individual making a report through the Hotline can choose one of three levels of anonymity:
- Remain completely anonymous to both the university and the Hotline
- Remain anonymous to the university, but not the Hotline
- Identify yourself
When reporters choose to remain anonymous only to UHD, university officials can communicate with the reporter through the Hotline to ask follow-up questions and give the reporter an update about the investigation.
Once your report has been submitted:
- You will receive a confidential access number.
- You will create a password and security question so you can anonymously check your report's progress through the Fraud & Non-Compliance Hotline.
- You can send and receive follow-up messages related to your report and take part in any additional steps if needed.
- To review the status of your report at any time, call 1-833-841-4512 or log in to the Fraud & Non-Compliance Hotline.

Reporting Sexual Misconduct
The Title IX Coordinator by law must review every report of sexual conduct. To report an incident directly to the Coordinator, rather than the Hotline, use the Sexual Misconduct Reporting Form:

Complainants
It's important for Complainants—those who were present during an incident—to include their contact information when submitting a Reporting Form, as their participation in the investigation is paramount. Complainants may withdraw their report at any time.
Reporters
Reporters—those who have heard of or witnessed someone else subjected to an incident on campus—have the option of submitting a Reporting Form anonymously; however, in that event, the Title IX Coordinator will not have a means by which to follow up with them for more information. The Coordinator will never disclose the identity of a reporter to an employee outside of the Title IX/Equal Opportunity Services department.
If a reporter would like to remain anonymous to the university but are open to responding to queries, they should submit their report through the Fraud & Non-Compliance Hotline, which can then act as an intermediary.
Supervisors who witness sexual harassment must report the harassment to the Office of Title IX/Equal Opportunity Services. Non-supervisory employees who witness sexual harassment are strongly encouraged to do the same.
