MOVE & SPACE REQUEST

A move Request form must be submitted to the Facilities Management department, when UHD  authorized employee is requiring the relocation of a staff/faculty member to another space. This relocation can include office to office moves, or cubicle to cubicle suite moves. The Move Request form must be submitted to the Facilities Management department before any Work-Order requests for moves are made.  After the Move Request form has been approved, a Work-order and Key request can be made to initiate the physical move.
 
Space Renovations/Adaptations
A Space Renovation/Adaptation form must be submitted to Facilities Management department, when an authorized UHD employee is requiring the renovation or alteration of a space for a specific purpose
The diagram shows the process the Space Renovation/Adaptation and Project forms will go through.  
 
Major Project Requests
A Project Request form is required when a department requires the major renovation or alteration of a space for a specific purpose. This form must be submitted to the Facilities Department before the major project can be initiated.
