Supervisor or Department Chair reviews and approves application form

Step 3

Department Head or Dean reviews and approves application form

(An automated process based on the online form filled out by faculty or staff)

Step 4

Appropriate Vice President reviews and approves application form

(An automated process based on the online form filled out by faculty or staff)

Step 5

Employee is notified once all approvals are complete

(An automated process)

Step 6

Employee authorized to perform consulting or paid professional service

Step 7

Approved application form filed in employee's official personnel file

Step 8

Employee completes UHS External Consulting Form annually as part of mandatory training

Employee will be notified via email of any Unapproved Application Forms (an automated process).
