Progressive Discipline Process - Staff - Employee Process Guide

Verbal Counseling

Step 1

Immediate Supervisor completes and signs Notice of Verbal Counseling documenting unsatisfactory work performance or misconduct.

Step 2

Immediate Supervisor discusses unsatisfactory work performance or misconduct and remedial action(s) expected with Employee.

Step 3

Employee signs Notice of Verbal Counseling form; form is maintained by Immediate Supervisor.

Step 4

Employee fulfills remedial action(s) within timetable or Immediate Supervisor initiates Written Reprimand.


Written Reprimand

Step 1

Immediate Supervisor completes and signs Employee Written Reprimand form documenting unsatisfactory work performance or misconduct not corrected and forwards to Department Head

Step 2

Department Head reviews and approves Employee Written Reprimand and forwards to Vice President, HR

Step 3

Vice President, HR reviews and approves Employee Written Reprimand and returns form to Immediate Supervisor

Step 4

Immediate Supervisor discusses unsatisfactory work performance or misconduct and consequences for inaction to improve with Employee

Step 5

Employee signs Written Reprimand; form is forwarded to Vice President, HR and placed in employee's official personnel file

Step 6

Employee fulfills remedial actions within timetable or Immediate Supervisor initiates Suspension without Pay


Suspension Without Pay

Step 1

Immediate Supervisor completes and signs Notice of Suspension Without Pay form and submits to Respective Vice President

Step 2

Respective Vice President reviews and approves Notice of Suspension Without Pay and forwards to Vice President, HR

Step 3

Vice President, HR reviews and approves Notice of Suspension Without Pay and returns to Immediate Supervisor

Step 4

Immediate Supervisor notifies Employee of suspension and employee's failure to demonstrate improvement will result in termination

Step 5

Employee signs Notice of Suspension Without Pay; form is forwarded to Vice President, HR and placed in employee's official personnel file

Step 6

HR initiates PeopleSoft ePAR to suspend Employee

Step 7

ePAR forwarded to appropriate levels for approval

Step 8

ePAR executed to suspend Employee

Step 9

Employee returns from suspension and fulfills remedial actions within timetable, or Immediate Supervisor initiates termination procedures. If Employee fails to return to work, Immediate Supervisor initiates termination procedures.


Dismissal from Employment

Step 1

Immediate Supervisor completes and signs Notice of Dismissal from Employment form and submits to Department Head

Step 2

Department Head reviews and approves Notice of Dismissal from Employment and forwards to Respective Vice President

Step 3

Vice President, HR reviews and approves Notice of Dismissal from Employment and returns form to Immediate Supervisor

Step 4

Immediate Supervisor and Vice President, HR notify Employee of dismissal and date and time of termination

Step 5

HR initiates PeopleSoft ePAR to terminate Employee

Step 6

ePAR forwarded to appropriate levels for approval

Step 7

ePAR executed to terminate Employee

Step 8

HR Benefits coordinates employee separation procedures with Employee


Contact HR Employee Relations for further information and guidance.

Immediate Supervisor will be informed of unapproved actions.

Supervisors and management may omit any stage under certain circumstances with review and approval by the Vice President, HR.

Employee will be notified of the employee's right to appeal reprimand, suspension or termination.
