Proof of Bacterial Meningitis
Texas law requires that all entering students under the age of 22 provide a certificate signed by a health care provider or an official immunization record verifying that a student has been vaccinated against bacterial meningitis, or has received a booster during the five years prior to registration.
UHD has chosen the services of Magnus Health SMR (Student Medical Record) to provide Web-based access of proof that a student has taken their Bacterial Meningitis vaccination. Proof of Meningitis Immunization must be submitted to Magnus Health SMR to verify that the vaccination was received during the five year period prior to and no later than 10 days before the first day of the semester.
UHD will not directly accept student Bacterial Meningitis Immunization records in person, to UHD fax numbers, or UHD email addresses. Please log onto UHD’s student services portal ‘myUHD’ to submit Bacterial Meningitis Immunization records for review by Magnus Health SMR (Student Medical Records). All students under the age of 22 are required to register through Magnus Health SMR, to ensure your course registration at UHD is not delayed. If you need assistance submitting your documents to Magnus Health SMR (Student Medical Records) please call 1-877-461-6831.

Registering for Spring Term 2021
Students registering for classes in the Spring 2021 term are currently still required to upload and submit proof that the student has taken their Bacterial Meningitis vaccination to Magnus Health SMR.
If your vaccination record does not show your shot or booster dose was receive by the compliance date deadline your class schedule will be in danger of being dropped.
Due to pandemic levels remaining extraordinarily high in our community and to ensure a safe start to Spring 2021, all classes will be conducted 100 percent online through at least Feb. 1. This will include those scheduled as face-to-face, hybrid, and online-interactive sections. Therefore the updated bacterial meningitis vaccination compliance deadline dates are listed below.
