Bacterial Meningitis

UHD continues to monitor the coronavirus outbreak closely and will update this page as necessary.

Proof of Bacterial Meningitis

Texas law requires that all entering students under the age of 22 provide a certificate signed by a health care provider or an official immunization record verifying that a student has been vaccinated against bacterial meningitis, or has received a booster during the five years prior to registration.

UHD has chosen the services of Magnus Health SMR (Student Medical Record) to provide Web-based access of proof that a student has taken their Bacterial Meningitis vaccination.  DO NOT submit your documents to UHD directly.

How to Submit your Bacterial Meningitis records
Fully admitted students should submit vaccination documents to Magnus Health SMR. Follow the steps on the Instructions on Uploading Vaccination Record. All students under the age of 22 are required to register through Magnus Health SMR. You will not be able to register for classes without providing documentation or exemption for this vaccination. The vaccine must be received 10 days before the first day of the semester. Only fully admitted students have access to the “Student Records & Enrollment” tile and will be able to upload their records to Magnus Health. To check the status of your admission application, login to your myUHD Student Center.
