Financial Aid Forms Guidelines
Most financial aid forms are available to download. Many forms are interactive, and should be filled out online, printed, and submitted to the financial aid office with the appropriate signatures.

Unless otherwise indicated, most forms may be submitted by mail, fax, email, in person at the financial aid office, or in our financial aid drop box. The student’s name and UHD ID number should be on each page submitted.
Signatures must accompany all documents. An electronic signature is not sufficient to serve as a signature. Documents can be filled out online, but should be printed and signed manually. Failure to sign the form will result in the document being considered incomplete.
Forms which are received incomplete (i.e., missing signatures, supporting documents, etc.) will be marked incomplete on the student account. Incomplete documents will not be processed. The student can then contact the financial aid office to find out how to ensure that documents are complete and processed.
Certain citizenship and identity documents must be submitted in-person at the financial aid office. For proof of citizenship such as a passport, birth certificate, permanent residency card, or certificate of naturalization, an original document must be presented at the financial aid office so that a copy may be made to fulfill the requirement.
The Identity and Statement of Educational Purpose form must be submitted in person at the financial aid office. The student must present a valid photo ID, such as a driver’s license, passport, or other state-issued ID.
