Due to COVID-19 UHD precautions, the UHD VITA Office will not be seeing face-to-face appointments. For assistance please call the VITA Office at 713-221-5015 or email ​​ to request either a zoom (online) or phone appointment.

What To Fill Out 

What to Bring to Your VITA Online or Phone Appointment

For U.S. Citizens or Resident Aliens, bring a completed Intake/Interview and Quality Review Sheet (IRS Form 13614-C).pdf

  • W-2, 1099-MISC, etc. and all sources of income you received in the current tax year
  • Student, Fee Bills
  • Interest and dividend statements from banks (Forms 1099)
  • Total paid for daycare provider, provider's EIN or
  • Social Security cards for you, your spouse and dependents, or
  • ITIN assignment letter
  • Documentation of any donations made to U.S. charities
  • Property taxes paid on your home or personal property (vehicles) documentation
  • All Forms 1095, Health Insurance Statements and
  • Health Insurance Exemption Certificate, if received
  • Last year's federal returns, if you filed last year
  • Income transcripts from IRS, if applicable
  • Bank routing and account numbers for direct deposit
  • Married-filing-jointly: both spouses' presence to sign the required e-filing forms
  • A completed Intake/Interview and Quality Review Sheet - IRS Form 13614-C

Non-Resident Aliens:

  • Passport
  • Form I-94, Arrival and Departure Record
  • Photo ID for all taxpayers to be included on your tax return
  • Foreign status proof, if applying for an ITIN
  • Birth dates for you, your spouse and dependents on the tax return
  • W-2, W-2G, 1099-R,1099-Misc) from all employers
  • 1042-S, Foreign Person's U.S. Source Income Subject to Withholding
  • Nonresident Alien Intake and Interview Sheet​
