  • Upon submitting this form, contact the Student Activities front desk to confirm reservation.
  • The keys for the reserved room of choice can only be checked out by the organization's contact person (after submitting their school ID or driver's license) during office normal business hours.
  • If the keys are going to be checked out overnight, they should be turned in to the front desk staff the next day no later than 8:30am.
  • You will be contacted via email/phone by the office staff, confirming or denying your reservation
  • The Office of Student Activities reserves the right to change or cancel room reservations without notice prior to a scheduled meeting as a result of Departmental priorities.
