Release of Student Records

The Registrar's Office maintains all student academic records, issues transcripts & diplomas, along with providing information for degree and enrollment verifications and loan deferments. Veterans benefits are also certified in the Registrar’s Office.

 

To request an official transcript, please follow the procedures indicated in the UHD Transcripts page.

 

For information regarding an Enrollment or Degree Verification, please follow the procedures indicated in the Enrollment & Degree Verification page.

 

To order a UHD Diploma, please complete a Diploma Order Form and follow the instructions listed on the form. 

 

To change your name or provide/update your Social Security Number, please complete a Name/SSN Change Form.

 

To request the release of your UHD ID please follow the instructions listed in the Look Up UHD ID page.

 

To reset your PIN/Password, please follow the instructions on the Forgotten Your PIN page.

 

To register for classes, please follow the enrollment steps listed in the Register For Classes page.

 

To audit a course, please complete a Request to Audit Form prior to the Day of Record of the academic term you are attending.
