Medical Records

All information is considered confidential and will not be released without the patient's written consent or a signed court order.

Patients have the right to obtain copies of their medical record.

 

General Requests for Medical Records Will Be Processed at No Charge

• Current Lab Results
• Current Clinician Notes
• Current Nurse's Notes

 

Requests for Complete Medical Records

• Mailed request should include a check or money order in the amount of $20.00, payable to UH-Downtown, and a copy of your Driver's License or State ID.
• Request made in person may be paid by cash, check or credit card.
• Authorized requests for complete medical records from another health care facility will be processed at no charge. Forms may be sent by mail or fax. Records will be mailed directly to the health care facility initiating the request.
