Campus Programs for Minors - Conducted by UHD

 

A Program Operator may not employ an individual in a position involving contact with a campus program for minors conducted by UHD unless:

 

 
  • The individual successfully completes the UHS Campus Program for Minors training and examination program on sexual abuse and child molestation, and documentation is available to verify successful completion.
   
 
  • The individual has successfully completed a criminal background check.

 

All of the following steps must be completed by the Program Operator prior to conducting any Campus Programs for Minors, regardless of the location of the program, that is, whether the program is on-campus or at an off-campus site.

 

Step 1.

At least 21 days before the program begins:

 

 
   
 
  • Collect Security Sensitive Questionnaires (SSQ) for all program employees, prospective employees and volunteers. [Note: Under "Please select report(s)" check "Criminal Background Check".]

    This document contains private information pertaining to the individual. It should be secured at all times and hand delivered to the VP of Employment Services and Operations (Ivonne Montalbano) or the Records & Business Administrator (April Frank).

 

  • Bring a hard copy of the completed "Spreadsheet" along with SSQ’s for all program participants to Ivonne Montalbano or April Frank.

 

Step 2.

At least 7 days before the program begins:

 

 
  • Ensure that all individuals listed on the spreadsheet submitted in "Step 1." have completed the Sexual Abuse and Child Molestation Training.

 

  Register all program participants in required training using the UHS Training Information Sheet. (This information sheet contains detailed information regarding enrollment, login information, troubleshooting and how to determine if you have passed the test. An individual must score 100% to pass the test and can take the test until the required score is received.)
   
  For volunteers, you must first,
 

 

Step 3.

At least 7 days before the program begins:

 

 

Complete the Department of State Health Services - Campus Program for Minors Information Form.

 

 
  • ESO will provide the results of the criminal background check and the dates each individual successfully completed the UH System training. Attach this information to the form.
  • The course information needed to complete the form is as follows:
    • UH System Training Course Name: University of Houston System Child Protective Training

      UH System Course Approval Number: CPM12-0072

    • Harris County Code: 101

Forward the completed Department of State Health Services - Campus Program for Minors Information Form and supporting documentation to the Coordinator of Campus Programs for Minors (Mary Cook at CookM@uhd.edu).

 

 

Keep a copy of the entire file with all supporting documentation for a period that is to be determined and will be communicated at a later date.

The Program Operator must obtain written approval from the Coordinator of Campus Programs for Minors to conduct the program.

 

 

Written Approval from the Coordinator of Campus Programs for Minors.

The Coordinator of Campus Programs for Minors will:

  • Review the Department of State Health Services - Campus Program for Minors Information Form and supporting documentation package submitted by the Program Operator. [Note: Incomplete packages will be returned to the Program Operator for correction/completion.]
   
 
  • Submit the Department of State Health Services - Campus Program for Minors Information Form to the Texas Department of State Health Services (TDSHS).
   
 
  • Provide written and dated notice to the Program Operator of approval to conduct the program.
