Documentation Guidelines

In order to determine eligibility and provide accommodations, ODS will need documentation. The type of documentation required may depend upon the type of disability and the specific accommodations a student would like to request.

To be eligible for disability services, a student must submit disability documentation (as described below in each link);

ADHD/ADD

Blind/Low Vision

Deaf/ Hard of Hearing

Head Injury/Traumatic Brain Injury

Learning Disability

Physical and Systemic Illness(Health)

Psychological Disability

Rehabilitation Drug/Alcohol Addiction
