Citizens'
Exchange of Accident Information- Use this form to gather and document
driver information when involved in a minor accident. Print the form and
complete form for your records and/or follow-up information for UH-D Police.
Motorist Agreement-
Use this form to expedite the process of having a motorist assist completed.
Open the form, fill-in the requested information, print the form and then
sign it. Call the Police, explain the nature of your motorist assist (sorry,
flat tires are not included) and location of your vehicle. Explain that
you have completed the form and will give it to the responding officer.
Parking Citation Appeal-
Use this form to appeal Parking Citations. Open the form, fill-in the
requested information, print the form and then sign it. Bring the completed
form (along with the original Parking Citation) to Parking Services
for processing.
Sworn Affadavit- Use
this form to begin the process of a complaint or in response to giving
a sworn statement. Open the form, fill-in the requested information and
then print the form. DO NOT SIGN THE FORM. The form
MUST be signed in the presence of a notary for document
authentication. Bring the form to the Police Department for presentation.
UH-D Police has an authorized notary public on all tours of duty.
Texas Public Information Act Request
Form- Use this form to make a request
of the UH-Downtown Police Department regarding an incident report, accident
report, information report or other information. Open the form, fill-in
the requested information and then print the form. Make sure to sign the
form. In order to ensure that your request is a valid request under the
Texas Public Information Act, please hand-deliver or mail the completed
form to the UH-Downtown Police Department.
