The Student Accident and Sickness Insurance Plan is endorsed by the University
of Houston-Downtown for Domestic and International Students

General Information: 
STEP 1:
Take the time to read and understand the policy prior to enrollment including exclusions, pre-existing condition clause and coverage limitations.
The student must first use the resources of Student Health Services where treatment will be administered or an outside medical referral will be issued.
Be responsible for your healthcare.  Know what to do in the event of a medical emergency or urgent care when Student Health Services is closed.
Eligible fees for Student Health Services are billed directly to the insurance company for students who are enrolled in the Plan.  Office visit fees, co-pays, and non-eligible expenses will be transferred to a student's account.
Examples of non-eligible services:  See plan for list of exclusions
Routine Pap Smear
  Routine STD Screening
  Routine Lab Screening (cholesterol, glucose, etc.)
  Non-Oral Contraceptives
  Routine Immunizations
 
HPV Vaccine is covered on this policy
STEP 2:
Students enrolled in the Student Health Insurance Plan for the first time are required to complete a Patient Information and Authorization forms with Student Health Services no later than the 30th day of the current semester (12th day in Summer semester).
Students not submitting the required forms by the posted deadline will have a hold placed on their student records.
OPTION 1:
Stop by Student Health Services during regular business hours to complete required paperwork.
OPTION 2:
Students enrolled in Distance Education or Saturday only classes should contact our office for alternative arrangements.
Eligibility:
All domestic students attending the University of Houston-Downtown who are enrolled for 6 or more credit hours (3 hours for summer session) are eligible for coverage (see brochure for complete eligibility requirements).
Health insurance is mandatory for all International students.
If you do not meet the eligibility requirements of this Plan, or desire dependent coverage, please call Macori, Inc. at (281) 651-8787 for information on alternate insurance plans.

Prescription Medications:
  Prescription coverage is limited to medications available from the Student Health Services Pharmacy (Antibiotics and Oral Contraceptives), In-Hospital medications and Mandated Benefits covered medication.
 
Medical Emergencies:
  Student Health Services is an ambulatory facility and not equipped as an emergency facility.
  On-Campus Medical Emergencies:
  Come to Student Health Services or contact the UH-Downtown Police Department by using a Emergency Call Box or calling

  Off-Campus Medical Emergencies:
 

Call 9-1-1 or go to a local emergency room.  St. Luke's Hospital is recommended by the Student Health Insurance Plan.

  Read and understand the plan's provisions for medical emergencies, your responsibilities and required notification procedures.
  Know the emergency room or urgent care center in your area that is covered under the insurance plan.
