| General Information: |
| STEP 1: |
| Take the time to read and understand the policy prior to enrollment including exclusions, pre-existing condition clause and coverage limitations. |

The student must first use the resources of Student Health Services where treatment will be administered or an outside medical referral will be issued. |
|
• |
Be responsible for your healthcare. Know what to do in the event of a medical emergency or urgent care when Student Health Services is closed. |
|
• |
Eligible fees for Student Health Services are billed directly to the insurance company for students who are enrolled in the Plan. Office visit fees, co-pays, and non-eligible expenses will be transferred to a student's account. |
|

| STEP 2: |
| Students enrolled in the Student Health Insurance Plan for the first time are required to complete a Patient Information and Authorization forms with Student Health Services no later than the 30th day of the current semester (12th day in Summer semester). |
| Students not submitting the required forms by the posted deadline will have a hold placed on their student records. |
|
OPTION 1: |
Stop by Student Health Services during regular business hours to complete required paperwork. |
|
OPTION 2: |
Students enrolled in Distance Education or Saturday only classes should contact our office for alternative arrangements. |
|
|
|
| Eligibility: |
| All domestic students attending the University of Houston-Downtown who are enrolled for 6 or more credit hours (3 hours for summer session) are eligible for coverage (see brochure for complete eligibility requirements). |
| Health insurance is mandatory for all International students. |
| If you do not meet the eligibility requirements of this Plan, or desire dependent coverage, please call Macori, Inc. at (281) 651-8787 for information on alternate insurance plans. |
