Do I need an appointment to receive Gardasil®?
Yes, you must make an appointment to be seen by a clinician.
1st Dose:  Initial Clinician Visit
You must have a current NORMAL Pap Smear on file, ask for a copy of your current Pap Smear to be sent to us or request a Pap Smear is obtain during your appointment.
You cannot be currently pregnant.  A pregnancy test may be required.
If Pap Smear is obtained during this visit, the vaccination will not be administered until the clinician has review the results.
2nd Dose:  2 Months
Clinician visit to discuss any changes in health or adverse reactions to the previous dose. 
3rd Dose:  6 Months
Clinician visit to discuss any changes in health or adverse reactions to the previous dose.

E.L.I. Student Health Insurance Does Not Have This Coverage

Students with private health insurance can request an itemize statement to send to their insurance company for reimbursement.

Does my insurance cover Gardasil®?

  • Gardasil® HPV Vaccine is covered by the Student Health Insurance Plan (Standard Policy). Co-pay required.  You must be 26 years of age or younger.
  • E.L.I. Students:  The Student Health Insurance policy for student enrolled in the English Language Institute does not cover Gardasil® HPV Vaccine.
  • Private Health Insurance:  Some policies do cover the Gardasil® HPV Vaccine.  Check your individual plan.  Students with private health insurance can request an itemize statement to send to their insurance company for reimbursement

Do I qualify for FREE Gardasil®?

If you do not have health insurance, or your health
insurance does not cover vaccines, you may be eligible
to receive Gardasil for FREE (a service fee may be required)

19 - 26 Years of Age:  May be eligible under the Merck Patient Assistance Program to receive FREE Gardasil® HPV Vaccine if :
You reside in the United States and are 19 years of age or older
You have no health insurance coverage
You have an annual household income of less than:
  $20,240 for individuals
  $27,380 for couples, or
  $41,300 for a family of four
  Special circumstances may be accepted

Clinics operated by State Universities are not eligible to participate in the Merck Patient Assistance Vaccine Program at this time. 

If you are eligible for this program, contact a local area physician's office, clinic or Planned Parenthood for more information.
