UHD IDs

Student, faculty, and staff IDs are now being issued by the office of Student Activities & Events. Renewal stickers are not issued anymore.

To get a new or replacement* ID, please bring the following during regular office hours:

Students:
-fee statement showing that an initial payment for the current or next semester has
been made
-photo ID

Faculty and Staff:
-ID request form from department. Download request form HERE.

*Please note that there is $4.00 fee for each replacement ID.
