All applications and application fees must be submitted to an Alpha Phi Sigma Officer or our Chapter Advisor, Dr. Belbot.

ONLY TYPED APPLICATIONS WILL BE ACCEPTED.

Note: Probationary application should be completed only by those seeking probationary membership. Please see special requirements on page 1.
