Sign Up Instructions and Contract Forms
- Fill out the participation contract
- Print off a copy of your UHD transcripts
(no need for official copies) - Take the deposit slip and pay $200 at the cashier’s window.
- Attach the copy of your receipt and transcripts to the participation contract.
- Slip all of these papers under Dr. LaRose’s office door (1036S)
- Wait for the confirmation email from Dr. LaRose
