UHD Trip to London and Edinburgh

You must have a passport by February 1, 2013 and passports must be valid through January of 2014. You will need to provide a copy of your passport to Dr. Jillian Hill as part of your travel documents.
Passport applications can take up to 2 months to process. You MUST start NOW! Do NOT wait!
If you are applying for the first time, you will need do the following:
- Fill out Form DS-11: Application For A U.S. Passport
- Submit completed Form DS-11 in person
- Submit evidence of U.S. citizenship
- Present identification
- Submit a photocopy of the identification document(s)
- Pay the applicable fee
- Provide one passport photo
Passport photos can be obtained at most FedEx Office locations and many drugstores (CVS, Walgreens).
After gathering the appropriate documents and completing the necessary paperwork, you will then visit an authorized passport office. Most often, authorized passport offices are located in post offices.
