The
Office of Community Relations and Conference Services coordinate the reservation
of campus facilities and aids in the management and providing of services
for conferences and events. Some of the services provided include
the reserving of facilities, setup of rooms, parking, catering, hotel
locations, police, and multimedia services. In order to request
facilities or services you must complete the appropriate request form.
Listed below are the various forms with a brief description of
the purpose and procedures. All forms must be submitted by hard
copy to the Community Relations and Conference Services offices located
at 621-S with the original authorized signatures on each appropriate request
form.
Reserve Tables/Display Area
If you need tables or a display area set up for the facility you are reserving
please complete the Request for
Table / Display Area
and submit it to the Office of Community Relations and Conference
Services in room 621-S.
Reserve a Campus Facility or Event
If you need to reserve a facility or plan an event on campus please complete
the Event Request Form and
submit it to the Office of Community Relations and Conference Services
in room 621-S. All events held on UHD campus by an approval UHD
student organization must be attended by the Student faculty / Staff advisor
at all times. Access to all facilities shall be granted only to
Faculty/Staff Advisors.
Request a Security Access Code
A number of rooms are kept locked due to
room features & equipment. Should you need to use or reserve
a lecture hall, or the auditorium, please complete the Security
Access Code Form, and submit by e-mail to Janet Heitmiller at HeitmillerJ@uhd.edu
or deliver to Community Relations and Conference Services in room 621-S.
Request
for Parking Services
If you need to reserve parking for your guest on campus you must complete
a Parking Request Form
and submit it to the Office of Community Relations and Conference Services
in room 621-S. All parking requests must be accompanies by
an SCR if you intend to guarantee a reserved parking space. No
parking spaces will be reserved without a completed SCR attached to the
request form.
Request
for Police/Security Service
All
requests for Police / Security services are coordinated through our office.
To request Police Officer (s) be scheduled for your event you must
complete a
Request for Police
/ Security Service form and submitted to our office no less than ten
(10) days prior to the date of your event. A completed SCR must
accompany your Request Form at the time of submission to our office.
When Officers are assigned a, confirmation with all pertinent information
will be sent to Requestor.
Application for Alcohol Permit
If you plan to serve alcohol at an event held in one of UH-Downtown's
facilities you must complete an Application
for Alcohol Permit and submit it to the Office of Community Relations
and Conference Services in room 621-S. This application must
be submitted no later than 30 days prior to the event.
Reserve Multimedia Equipment
If you need any multimedia or instructional television equipment for an
event held in one of UH-Downtown's facilities you must complete a Multimedia
Reservation form and submit it to the Multimedia and ITV Services
office in room 700-A. You must also make note on your Request
for Campus Facilities Use form that you are requesting multimedia equipment
for your event.
Request for Temporary Food Dealer's Permit
If you are an organization, department or group selling, distributing
or serving any food or beverage not served by the authorized campus food
provider you must complete a Temporary
Food Dealers Permit / Request Form. You must read, understand
and agree to the policies and procedures outlined in the Food
Sanitation Manual prior to submitting this form. This form is
to be filled out completely, signed and approved by the Safety Inspector
at UHD, located in the Facilities Management Department located in room
101-South. You may reach him at (713) 221-8040. Once completed,
it must be attached to your Event Request Form and submitted to the Community
Relations and Conference Services office at least 7 working days prior
to the date of your event for approval and confirmation.
